- Facility Questionnaire: Part I is required for IMRT on this protocol.

RPC Phantom Dosimetry Test
- Fill out the RPC Phantom Request Form to request shipment of a phantom.
- Contact Bill Straube at (314) 362-9762 regarding submission of phantom treatment planning data to the ITC.
- Credentialing - Brachytherapy
Institutions wishing to enter patients onto RTOG protocols that include brachytherapy implants must be credentialed by the Radiological Physics Center (RPC) and demonstrate the ability to submit brachytherapy data electronically to the Image-Guided Therapy QA Center (ITC). Credentialing is for specific personnel, nuclide, manufacturer and model of seed, and treatment planning computer. If anything has changed since your initial credentialing or you wish to use additional seed models, please refer to the RPC website for additional information.

PLEASE NOTE: As of 2/4/2014, this protocol has been amended to require treatment planning data submission via the TRIAD system. Cases with case numbers above 153 are to be submitted via TRIAD at RTOG HQ. Please see section 12.2 of the protocol for information on TRIAD.
